ABSTRACT

Resumo A doença de Lyme é uma infecção sistêmica causada pela espiroqueta Borrelia burgdorferi e transmitida pelo carrapato do gênero Ixodes sp. e espécie Amblyomma cajennense. A doença costuma se manifestar em três estágios clínicos distintos, que podem variar de acordo com as características de cada hospedeiro. O objetivo deste trabalho é relatar o caso de uma paciente de 33 anos com doença de Lyme que apresentou como manifestações neuroftalmológicas diplopia, lagoftalmo paralítico e ceratite punctata, com exames laboratoriais negativos. Embora a espiroqueta Borrelia burgdorferi tenha maior tropismo pelos tecidos da pele, sistema nervoso e articulações, o acometimento ocular não deve ter sua importância diminuída, sendo descrito neste relato de caso, que abordou os aspectos mais pertinentes à doença para auxiliar seu diagnóstico e tratamento.


Abstract Lyme disease is a systemic infection caused by the spirochete Borrelia burgdorferi and transmitted by the tick of the genus Ixodes sp. and species Amblyomma cajennense. The disease usually manifests itself in three distinct clinical stages, which may vary according to the characteristics of each host. The objective of this paper is to report the case of a 33-year-old patient with Lyme disease who presented as neuro-ophthalmological manifestations diplopia, paralytic lagophthalmos and punctate keratitis, with negative laboratory tests. Although the spirochete Borrelia burgdorferi has a greater tropism in the tissues of the skin, nervous system and joints, the ocular involvement should not be diminished, being described in this case report, which approached the most pertinent aspects to the disease to aid its diagnosis and treatment.

ABSTRACT

A espiroquetose aviária é uma enfermidade septicêmica de curso agudo, cosmopolita, que acomete diversas espécies aviárias, causada por Borrelia anserina e transmitida pelo carrapato Argas miniatus. O experimento teve como objetivos avaliar as alterações bioquímicas e anátomo-histopatológicas no fígado de Gallus gallus, causadas pela infecção experimental por B. anserina. Quarenta aves da espécie G. gallus foram divididas em quatro grupos inteiramente casualizados com 10 animais cada: G1 - inoculado com soro infectado com B. anserina; G2 - inoculado com soro fisiológico a 0,9%; G3 - exposto a ninfas de terceiro ínstar de A. miniatus infectados por B. anserina; G4 - exposto a ninfas de terceiro ínstar de A. miniatus livres de B. anserina. As aves dos Grupos 1 e 3 manifestaram no 3º e 6º dias pós-inoculação (DPI) respectivamente, sintomatologia característica da doença como inapetência, perda de peso, sonolência, diarreia esverdeada, mucosas hipocoradas, penas arrepiadas e hipertermia. Os níveis de ALT do Grupo 1 mostraram-se significativamente mais elevados apenas no 12ºDPI e 24ºDPI em relação ao seu grupo controle (Grupo 2) e no Grupo 3 esses níveis se mantiveram elevados até o 20º DPI em comparação ao seu grupo controle (Grupo 4). Os níveis da enzima AST pouco oscilaram nos grupos experimentais, embora tenham sido encontradas elevações no 12ºDPI nos Grupos 1 e 3. Os fígados das aves dos Grupos 1 e 3 apresentaram à necropsia, moderada hepatomegalia, congestão, superfície irregular e coloração vermelha a cianótica; constataram-se ainda pequenos pontos esbranquiçados na superfície. A histopatologia do fígado revelou congestão, infiltrados inflamatórios mononucleares, focos de necrose fibrinoide, dilatação dos sinusoides e vacuolização de hepatócitos. A coloração de Warthin-Starry revelou, nos fígados das aves dos Grupos 1 e 3, a presença de espiroquetas compatíveis com B. anserina, frequentemente no interior de vasos sanguíneos.(AU)


Spirochetosis avian is a septicemic disease of acute course and cosmopolitan can affect various avian species, caused by Borrelia anserina and transmitted by Argas miniatus. The experiment aimed to evaluate the biochemical, anatomical and histopathological changes in the liver of Gallus gallus caused by experimental infection with B. anserina. A total of 40 fowls of the species G. gallus were divided into four randomized groups of ten fowls each: G1 - inoculated with serum infected with B. anserina; G2 - inoculated with 0.9% saline; G3 - exposed to nymphs of 3rd instar of A. miniatus infected with B. anserina; G4 - exposed to ticks nymphs of 3rd instar of A. miniatus free of B. anserina. The fowls of Groups 1 and 3 expressed at 3 and 6 days post-inoculation (DAI) respectively , symptoms characteristic of the disease as lack of appetite , weight loss , drowsiness, greenish diarrhea, pale mucous membranes , ruffled feathers and hyperthermia. ALT of group 1 levels were significantly higher only at the 12º and 24º day after inoculation (DAI) compared with its control group (group 2), and in group 3 these levels remained high until the 20º DAI as compared with its control group (group 4). AST enzyme fluctuated little in the experimental groups, although elevations at 12ºDAI has been found in group 1 and 3. The liver of fowls in groups 1 and 3, presented at necropsy moderate hepatomegaly, congestion, irregular surface and red color to cyanotic. If found even small whitish spots on the surface. The histopathology revealed congestion, mononuclear inflammatory infiltrates, fibrinoid necrotic foci, dilatation of sinusoids, and vacuolation of hepatocytes. The Warthin-Starry staining revealed in the liver of fowls in groups 1 and 3 the presence of spirochetes compatible with B. anserina, often within blood vessels.(AU)

ABSTRACT

Infectious disease disasters are events that involve a biological agent, disease and that result in mass casualties, such as a bioterrorism attack, an emerging outbreak of infectious disease; all disasters pose a risk of infection transmission. But, infectious disease disasters pose the great-risk to illness or death from an infectious disease. This study raised the awareness and improved knowledge by educational program for Military Nursing Staff on selected infectious disease disasters acquired at Egyptian Eastern Border. The selected arthropodborne diseases were Anthrax, Tick borne relapsing, Louse borne replasing fever and liver fluke; Clonorchis sinensis. An interventional study was used, for 125 staff nurse who accepted to participate. The tools dealt with four questionnaires: [1] Some socio-demographic characteristics data [2] Educational needs assessment a structured questionnaire. [3] Knowledge test [pre/ post-test] and [4] Participants' reactions questionnaire. The results showed that educational intervention significantly improvements the nursing staff knowledge, which were achieved at the immediate post intervention phase, and retained via three months post-test phase. In the service training programs about infectious disease disasters at Egyptian Eastern Border must be established and continued on regular basis. This would improve their knowledge about the epidemiology of these infectious disease disasters

ABSTRACT

Introducción: Borrelia burgdorferi sensu lato es el agente etiológico de la enfermedad de Lyme, zoonosis emergente de difícil diagnóstico, prevención y control, reportada fundamentalmente en el hemisferio norte. Objetivo: facilitar información actualizada acerca de Borrelia burgdorferi. Métodos: se realizó una revisión de la literatura científica y especializada sobre los principales aspectos relacionados con este agente y su enfermedad; como son las características de las borrelias y el ciclo de vida, epidemiología, manifestaciones clínicas en humanos, diagnóstico de laboratorio, definición actual de caso, tratamiento, profilaxis, prevención y control. Resultados: se expone información actualizada y valiosa sobre los temas seleccionados, útil para el personal interesado en las enfermedades infecciosas transmitidas por vectores. Se muestra además información sobre los estudios realizados en Cuba. Conclusiones: se aporta información de utilidad para el manejo epidemiológico, clínico y microbiológico de casos con sospechas de la infección por Borrelia burgdorferi sensu lato


Introduction: Borrelia burgdorferi sensu lato is the causative agent of Lyme disease, an emerging zoonosis, whose diagnosis, prevention and control are difficult and it is mainly reported in the northern hemisphere. Objective: to provide updated information about Borrelia burgdorferi. Methods: a review of scientific and specialized literature on the key aspects of this agent and the disease such as characteristics and life cycle of borrelias, epidemiology, clinical manifestations in humans, laboratory diagnosis, current case definition, treatment, prophylaxis, prevention and control. Results: current valuable information on selected items was set forth, which is useful for the personnel involved in vector-borne infectious diseases. Additionally, information about studies conducted in Cuba was provided. Conclusions: this paper offers updated information for the epidemiological, clinical and microbiological management of suspected cases of infection with Borrelia burgdorferi sensu lato

INTRODUCTION: The symptoms of Brazilian borreliosis resemble the clinical manifestations of Lyme disease (LD). However, there are differences between the two in terms of epidemiological and laboratory findings. Primers usually employed to diagnose LD have failed to detect Borrelia strains in Brazil. OBJECTIVE: We aimed to identify the Brazilian Borrelia using a conserved gene that synthesizes the flagellar hook (flgE) of Borrelia burgdorferi sensu lato. METHOD: Three patients presenting with erythema migrans and positive epidemiological histories were recruited for the study. Blood samples were collected, and the DNA was extracted by commercial kits. RESULTS: The gene flgE was amplified from DNA of all selected patients. Upon sequencing, these positive samples revealed 99% homology to B. burgdorferi flgE. CONCLUSION: These results support the existence of borreliosis in Brazil. However, it is unclear whether this borreliosis is caused by a genetically modified B. burgdorferi sensu stricto or by a new species of Borrelia spp.


INTRODUÇÃO: Os sintomas da borreliose brasileira se assemelham às manifestações clínicas da Doença de Lyme (DL), porém, existem diferenças epidemiológicas e laboratoriais entre essas enfermidades. Primers normalmente utilizados para diagnosticar a DL não conseguiram detectar cepas de borrelia no Brasil. OBJETIVO: O objetivo desse trabalho foi identificar a borrelia brasileira usando um gene conservado que sintetiza o gancho flagelar (flgE) da Borrelia burgdorferi sensu lato. MÉTODO: Três pacientes com eritema migratório e epidemiologia positiva foram recrutados para o estudo. Amostras de sangue foram coletadas, e o DNA foi extraído por kits comerciais. RESULTADOS: O gene flgE foi amplificado a partir do DNA de todos os pacientes selecionados. Após o sequenciamento, essas amostras positivas revelaram homologia de 99% para B. burgdorferi. CONCLUSÃO: Estes resultados reforçam a existência de borreliose no Brasil. No entanto, não está claro se esta borreliose é causada por uma variante geneticamente modificada da B. burgdorferi sensu stricto ou por uma nova espécie de Borrelia spp.

Relapsing fever caused by Borrelia persica, is an acute tick-borne disease which is transmitted by soft ticks of Ornithodoros tholozani to human. Value of PCR and xenodiagnosis for detection of B. persica in O. tholozani ticks was compared. Sixty-four Borrelia- free ticks were fed on infected guinea pigs and used for the experiments. For xenodiagnosis, a group of 32 ticks in subsequent blood meal were fed on sterile guinea pigs and the indication of B. persica in the animal blood was tested 5-14 days later by dark-field microscopy. For PCR, all 64 ticks were subjected to PCR against B. persica rrs gene [16S-rDNA]. Also sensitivity of PCR in terms of minimum detectable number of spirochetes as well as the effects of tick sex and post digestion was tested. PCR revealed B.persica DNA in 98.4% ticks, in which B. persica were found in 25.0% by xenodiagnosis. PCR was enough sensitive to give positive results for DNA of 1 spirochete. PCR success rates were similar for male or female ticks. Course of time did not affect the efficacy of PCR and similar results were observed for ticks of immediately fed, semior completely gravid or completely digested blood ones. Our results indicate that due to very low specificity and time consuming, xenodiagnosis is not a useful method whereas PCR method has advantages for study the Borrelia prevalence in ticks

ABSTRACT

The aim of this study was to evaluate the epidemiological, clinical and laboratorial profile of patients with Baggio-Yoshinari Syndrome (BYS), who underwent internment at the Instituto de Infectologia Emilio Ribas in São Paulo, Brazil, during the period from July 1990 to July 2006. BYS is a new Brazilian tick-borne disease caused by Borrelia burgdorferi sensu lato microorganisms that resembles features of Lyme disease (LD), except for its epidemiological, clinical and laboratorial particularities. From 60 patients' records with positive serology to B. burgdorferi done by ELISA and Western-blotting methods, 19 cases were diagnosed as having BYS, according to criteria adopted at LIM-17 HCFMUSP, the Brazilian Reference Laboratory for the research of BYS. The other 41 remaining patients displayed miscellaneous infections or auto-immune processes. The beginning of symptoms in BYS group varied from one day to six years, from the onset of the disease. Four of 19 patients were included in acute disease stage, and 15 in latent. General unspecific symptoms were identified in almost all cases, with high frequencies of fever (78.9 percent) and lymphadenomegaly (36.8 percent). Six patients had skin lesions (31.5 percent); six arthralgia or arthritis (31.5 percent) and eight neurological symptoms (42 percent). Interestingly, two patients showed antibodies directed to B. burgdorferi exclusively in cerebrospinal fluid. Since BYS is a new emergent Brazilian zoonosis and its diagnosis is sometimes complex, all the new knowledge about BYS must be scattered to Brazilian Medical specialists, aiming to teach them how to diagnose this amazing tick-borne disease and to avoid its progression to chronic irreversible sequels.


O objetivo do estudo foi avaliar o perfil epidemiológico, clínico e laboratorial dos pacientes com Síndrome Baggio-Yoshinari (SBY), internados no Instituto de Infectologia Emilio Ribas, São Paulo, Brasil, no período de julho de 1990 a julho de 2006. SBY é uma nova doença transmitida por carrapatos, causada pela Borrelia burgdorferi sensu lato, que se assemelha a Doença de Lyme (DL), exceto pelas particularidades epidemiológicas, clínicas e laboratoriais. A partir dos registros de 60 pacientes com sorologia positiva para B. burgdorferi pelos métodos de ELISA e Western-blotting, 19 casos foram diagnosticados como SBY, de acordo com critérios adotados pelo LIM-17 HCFMUSP, laboratório de referência para a pesquisa de SBY no Brasil. Os outros 41 pacientes restantes foram classificados como tendo infecções diversas ou processos auto-imunes. O início dos sintomas no grupo SBY variou de um dia a seis anos. Quatro dos 19 pacientes foram incluídos na fase aguda da doença e 15 na fase latente. Os sintomas gerais inespecíficos foram identificados em quase todos os casos, com altas freqüências de febre (78,9 por cento) e linfoadenomegalia (36,8 por cento). Seis pacientes tiveram lesões de pele (31,5 por cento); seis artralgia ou artrite (31,5 por cento) e oito sintomas neurológicos (42 por cento). Curiosamente, dois pacientes apresentaram anticorpos para B. burgdorferi exclusivamente no líquido cefalorraquidiano. Uma vez que a SBY é uma zoonose emergente brasileira e seu diagnóstico é complexo, todo o conhecimento novo sobre SBY deve ser difundido para os médicos brasileiros, com o objetivo de ensiná-los a diagnosticar esta surpreendente doença transmitida por carrapatos, evitando-se a sua progressão para sequelas crônicas irreversíveis.

A ocorrência de manifestações clínicas e laboratoriais semelhantes às encontradas na doença de Lyme e da coinfecção com a babesiose já foi demonstrada em trabalhos anteriores em adultos, porém não existem estudos desta natureza em crianças. OBJETIVO: Caracterizar o perfil clínico-epidemiológico da síndrome de Lyme-Símile em crianças do Estado de Mato Grosso do Sul e avaliar a prevalência dos anticorpos anti-Borrelia burgdorferi e anti-Babesia bovis no soro de pacientes que preencheram os critérios estabelecidos. MÉTODOS: 100 pacientes entre 9 meses e 16 anos de idade foram submetidos à pesquisa dos anticorpos pela técnica de ELISA e a soroprevalência foi comparada com um grupo-controle. RESULTADOS: Positividade para anticorpos anti-B. burgdorferi ocorreu em 27 por cento dos pacientes com suspeita clínica, sendo 17 por cento IgM e 12 por cento IgG. As manifestações articulares ocorreram em 21 pacientes, as manifestações cutâneas em três pacientes e as manifestações neurológicas em três pacientes. A prevalência de anticorpos contra B. burgdorferi nos pacientes do grupo-controle foi de 15 por cento (p < 0,05). Os anticorpos anti-B. bovis estiveram presentes em 24 por cento dos pacientes suspeitos e em 3 por cento do grupo-controle (p < 0,05). Concomitância dos anticorpos ocorreu em 10 pacientes com suspeita clínica e em nenhum controle. CONCLUSÃO: Os resultados deste estudo evidenciam a etiologia de caráter infeccioso e reacional das manifestações clínicas encontradas nos pacientes, podendo caracterizar uma síndrome, que possivelmente ocorra na presença de múltiplos microrganismos e está relacionada com carrapatos. Foi encontrada freqüência superior de anticorpos contra Borrelia e Babesia nos pacientes em relação aos controles. Embora pequena, essa diferença foi estatisticamente significante. Este achado não atesta que o quadro clínico dos pacientes seja causado por espécies de Borrelia ou Babesia, mas levantam a possibilidade de participação...


The occurence of clinical and laboratory manifestations similar to the Lyme disease and the coinfection with babesiosis was already demonstraded in previous researches with adult patients. However, there are no studies in children. OBJECTIVE: To identify the clinical-epidemiological profile of the Lyme Simile Syndrome in children of the State of Mato Grosso do Sul and to evaluate the prevalence of antibodies against Borrelia burgdorferi and Babesia bovis in the serum of patients that had fulfilled the established criteria. METHODS: One hundred patients (age range: 9 months and 16 years-old) were screened for the presence of those antibodies, using ELISA, comparing to a control group. RESULTS: Antibodies against B. burgdoferi were found in 27 percent of the patients with a clinical picture. The articular manifestations occurred in 21 patients, cutaneous in 3 and neurological in 3. The prevalency in the control group was 15 percent (p < 0.05). The antibodies for Babesia bovis were present in 24 percent of suspect patients and in 3 percent of the control group (p < 0.05). Concomitance of the two antibodies occurred in 10 patients with clinical suspect and no one of control group. CONCLUSION: The results found in this study suggest the infective and reactive character of the clinical manifestations of the patients, which can characterize a Syndrome, which possibly occurs in the presence of multiple microorganisms that can be associated with tick-borne. It was found a higher frequency of antibodies against borrelia and babesia in patients in relation to the controls. Although small, this difference was statistically significant. This finding does not certify that the clinical features of patients is caused by species of borrelia or babesia, but raised the possibility of involvement of some infectious agent that could be related to these micro-organisms. ABSTRACT

La artritis de Lyme persistente en niños se define como la evolución terciaria de la borreliosis de Lyme, localizada únicamente en un órgano específico. Simula la artritis idiomática juvenil oligoarticular, que con anterioridad se relacionaba con una predisposición inmunogenética, con mayor prevalencia del alelo del complejo mayor de histocompatibilidad MHC II DRB1*1104. Esta conexión se utilizó para rastrear nuestros pacientes con artritis persistente de Lyme. Se proponen las características clínicas salientes en relación con la literatura existentes; en la revisión se resumen las correlaciones con nuevos descubrimientos de inmunología clínica y básica relacionados con ambas patologías. Se mencionan los factores ecológicos que influyen en el aumento de la prevalencia de la borreliosis de Lyme y se enumeran las hipótesis acerca de la persistencia de Borrélia en los seres humanos. Se proponen y revisan nuevos datos obtenidos a partir de la investigación en la enfermedad de Lyme y en enfermedades autoinmunes relacionadas, con atención a una posible explicación para las diferencias individuales en el procesamiento del microorganismo responsable y para la diferente evolución de la enfermedad

ABSTRACT

A descriptive study was conducted at Combined Military Hospital Rawalakot, Azad Kashmir to determine the frequency of Borrelia recurrentis. Both out door and indoor cases were included. A total of 80 febrile patients with temperature above 1000C were studied. Cases showing any other cause of fever on routine / available tests were excluded. Borrelia recurrentis was sought by Giemsa stain. All positive cases were treated with Doxycycline. Nine cases were positive [11.25%]. Treatment response was 100%. Borrelia recurrentis is found in a significant number of febrile cases at Rawalakot Azad Kashmir

Objetivos: Detectar anticuerpos IgG/IgM contra Borrelia burgdorferi en población general, procedentes de los departamentos de Piura y Amazonas e identificar especies de garrapatas probablemente incriminadas en la transmisión de la enfermedad de Lyme. Material y Métodos: Entre agosto del año 2001 y junio de 2002, se colectaron muestras de sangre de 232 pobladores procedentes de ocho localidades del Departamento de Piura y 12 del Departamento de Amazonas, para evaluar mediante ELISA CaptiaTM Lyme IgG/IgM (Trinity biotech) la presencia de anticuerpos contra Borrelia burgdorferi. Además, se colectaron garrapatas en animales domésticos por búsqueda directa. Resultados: Se detectó seropositividad en 9,9 por ciento de los sueros evaluados. Asimismo, de 433 garrapatas colectadas se identificaron los géneros: Ixodes (5,5 por ciento), Amblyomma (18,0 por ciento), Rhipicephalus (23,5 por ciento), Anocentor (31,1 por ciento) y Boophilus (21,7 por ciento). Conclusiones: Existen personas seropositivas por Borrelia en Piura y Amazonas, coincidiendo con los hallazgos realizados en Sapillica en el año 1992, además se detectó la presencia de garrapatas del género Ixodes en Piura.

ABSTRACT

The seasonal dynamics of Ornithodoros [O.] savignyi and infection with Borrelia sp. in tick, domestic animals and man were investigated. The tick population densities of adults and immatures were high during the period from June to October with maximum levels in August and September, respectively. Adults predominated [58.6% - 80.0%] in October to May with the highest abundance in January, while immatures prevailed [51.3% - 65.8%] during the rest of the year reaching a peak in September. Of 1202 tick examined, the spirochetes were detected in 427. The annual infection rate in adults and immatures varied from 34.4% to 36.9% with no significant difference among larvae, nymphs, males and females. Of 1396 sera from man and 553 sera from host animals tested; 309 and 157, respectively, were reactive for antibody to Borrelia sp. Antigen with the highest infection rate in camel [47.8%], followed by sheep [23.8%], goat [18.0%] and cow [16.0%], then the least in buffalo [10.9%]. Changes in prevalence of Borrelia infection in the tick population correlated positively with changes in antibody prevalence in man and camel and with changes in tick population density

ABSTRACT

Soros de aves experimentalmente infectadas, contendo espiroquetas viáveis, foram submetidos a dois procedimentos antes da criopreservaçäo: glicerol na diluiçäo de 1/2 (v/v), designado como soro com glicerol a 50 por cento (GS), e dimetilsulfóxido na proporçäo de 1/10 (v/v), designado como soro com DMSO a 10 por cento (DS). Após 15 meses de estocagem em nitrogênio líquido, amostras dos tratamentos GS e DS foram descongeladas e suas infectividades foram testadas em frangos susceptíveis. Apesar de ambos os procedimentos terem mantidos a infectividade da bactéria, DMSO a 10 por cento no soro de frango apresentou-se mais satisfatório como criopreservante

ABSTRACT

Objetivo: actualizar el rol de las borrelias, en relación con varios cuadros nosológicos de interés dermatológico de etiología oscura, que han sido estudiados recientemente y deben ser más conocidos por los dermatólogos prácticos. Se analiza la enfermedad de Lyme y se discuten brevemente la esclerodermia localizada, liquen escleroso y atrófico, fasceítis eosinofílica, atrofodermia de Pasini y Pierini, granuloma anular, paniculitis septal, nódulos fibrosos subcutáneos, enfermedad de Jessner-Kanof, la hemiotrofia facial de Parry-Romberg y el linfoma B cutáneo primario que parecen asociarse con borrelias. La enfermedad de Lyme es un cuadro infeccioso sistémico, causado por espiroquetas del género Borrelia, transmitidas al hombre por garrapatas de género Ixodes. Tiene diversas manifestaciones cutáneas a lo largo de cada una de las etapas por las que atraviesa el proceso, por ejemplo el eritema migrans y el linfocitoma cutis (LC) en el estadío temprano y la acrodermatitis crónica atrofiante (ACA) en el estadío tardío. Se acompaña de síntomas principalmente neurológicos y reumatológicos. Existe tratamiento antibiótico útil, con dixiciclina, amoxicilina o eritromicina. Respecto a las otras enfermedades enumeradas, la terapia antibiótica anti-borrelia es todavía de valor discutible. Conclusión: si las diferentes entidades asociadas con borrelias son marcadores cutáneos de la enfermedad de Lyme o simplemente hallazgos coincidentes, es algo que aún está por determinarse
